Suitability Screens
The Navy’s Suitability Screening Program is used to determine the suitability of Sailors and their dependents for overseas or remote assignments.
Needed for individuals who will be going operational, to a deployable unit, overseas, or remote assignments.
Complete within 30 days of receiving orders.
Must have hardcopy orders in hand to properly screen.
The orders must have dependents listed for overseas locations

Medical Records for the last 5 years (outside of MTF for any appointment) to include Well Adult exam or Well child check for children. *Within the last year*
(If there is on going medical care for any diagnosis PCM must fill out EFMP documents on the

Dental records and exam to include x-rays of bite wing and dental notes not the summary’s. *Within the last 6 months*
Steps for Suitability/Overseas Screening
Fill out the forms and gather the medical records required.
Submit them to the Suitability Screening Officer via email (
usn.kitsap.navhospbremertonwa.list.oss-op-screen-efm@health.mil) or in person. We are located on the 3
rd deck in Patient Administration.
The Suitability Screening Office will perform a record review. Once it has been completed, you will be contacted to schedule the suitability appointment.
